Kaiser Permanente caregiver authorization — For residents whose health insurance is Kaiser Permanente, we request to be added as an authorized caregiver on their account, allowing us to coordinate and request medications directly on their behalf. You can trust that their care is managed seamlessly, giving you true peace of mind.
Comprehensive daily vital signs monitoring & documentation — We maintain detailed logs of each resident’s daily vital signs and administered medications. For residents where bowel health is a concern, we also track and document bowel movements to ensure nothing goes unnoticed. When your loved one’s home health nurse visits, complete and accurate records are always readily available — supporting seamless continuity of care between our facility and their medical team.
Accessible transportation for all mobility needs — We have a partnered transportation service equipped to accommodate wheelchair-bound and bed-bound residents, ensuring your loved one can safely attend medical appointments and outings without the stress of arranging transport on your own.
Appointment escort service — When family members are unavailable to accompany their loved one to a medical appointment, our staff can provide a dedicated escort to ensure they are never alone. This optional service is available for an additional fee, giving families confidence that their loved one is supported and well-accompanied every step of the way.
In-home case manager for personalized care coordination — A dedicated case manager works closely with each resident, their family, and their healthcare providers to ensure individualized, consistent care.
In-home live piano music for resident enrichment — A professional pianist performs right in our home, weaving live music into daily life to uplift spirits, foster connection, and enhance the quality of life for every resident.
